Care Coordination

Care Coordination

Coordinating tailored solutions for the individual across multiple government and non-government service systems

The Partners in Recovery (PIR) program is for people who experience severe and persistent mental illness, and who have complex needs. PIR facilitates the development of a web of local supports for participants to ensure they are well supported in the community.

PIR is about connecting people with the services they require, ensuring that service is meeting the person's needs and that the relationship between the person and service provider is effective. PIR participants are linked with a Support Facilitator who works with them to access the services and supports they need. Support Facilitators work at various sites and organisations across Canberra.

Phone PIR on 02 6287 8070 or visit https://www.chnact.org.au/partners-in-recovery
